Keep each form question connected to its choices

A screen reader (software that reads the page aloud) can announce several Yes and No options without the question they answer. Visual spacing alone may make grouped questions understandable only to visitors who see the layout.

Why this matters to your practice

A page may look neatly grouped while sounding confusing when read aloud by screen-reading software. This software lets people listen to and navigate a page instead of relying on its appearance. Your supplier should demonstrate the actual questions and answers together. As the practice owner, listen for meaning: could someone distinguish permission to email from a preference for a callback? You do not need to choose the coding method, but you can recognise an ambiguous question.

How to check the result

Accept when someone navigating directly among form controls can identify the question, available answers and current choice for every group.

A mistake to avoid

Repeating the complete question inside every answer can become noisy; ask the supplier to use an appropriate grouping pattern.
